Giving patients with Type 2 diabetes a
combination of the selective dipeptidyl peptidase-4 inhibitor
saxagliptin and metformin early, rather than waiting until one drug
fails, gives superior long-term glycemic improvements compared with
either monotherapy alone, research suggests.
Andreas Pfützner, from the Institute for Clinical Research
and Development in Mainz, Germany, who presented the findings at
the American Diabetes 70th scientific sessions in Orlando, Florida,
USA, reported that these superior effects were sustained over a
76-week period.
Moreover, combined treatment led to a significant 51% of
patients receiving 5 mg or 10 mg of saxagliptin plus metformin
achieving the American Diabetes Association’s recommended glycated
hemoglobin (HbA1c) target of 7%, compared with 25% of patients
taking 10 mg of saxagliptin alone and 35% of those taking metformin
alone.
For the study, Pfützner and colleagues randomly assigned
1306 patients, aged 18-77 years, whose Type 2 diabetes had been
inadequately controlled on diet and exercise, to receive
saxagliptin 5 mg plus metformin 500 mg (n=320), saxagliptin 10 mg
plus metformin 500 mg (n=323), saxagliptin 10 mg plus placebo
(n=335), or metformin 500 mg plus placebo (n=328) for an initial 24
weeks followed by a 52-week long-term extension period.
The patients had a mean baseline HbA1c of 9.5% and analysis of
the data included patients who did and did not require rescue
therapy.
After the entire 76 weeks, complete data were available for 1240
patients, including 612 who completed the 76 weeks without needing
rescue therapy.
Patients receiving saxagliptin and metformin combined had a
significantly greater reduction in HbA1c from baseline than did
patients on either drug alone, at 2.31% with saxagliptin 5 mg and
2.33% with saxagliptin 10 mg versus 1.55% with saxagliptin alone
and 1.79% with metformin alone.
Significantly greater reductions in mean fasting plasma glucose
levels from baseline were seen with combined therapy, at an average
of 54 and 55 mg/dl with saxagliptin 5 and 10 mg plus metformin,
respectively, compared with just 24 mg/dl with 10 mg saxagliptin
alone and 40 mg/dl with metformin alone.
Pfützner reported similarly “impressive” superior
reductions in postprandial glucose with combined therapy with no
attenuation between weeks 24 and 76, whereas there was partial
attenuation in both monotherapy groups.
Saxagliptin and metformin combined treatment was generally well
tolerated, with the frequency of adverse events low overall, and
similar across the four treatment groups, at 65.9% for saxagliptin
5 mg plus metformin, 68.4% for saxagliptin 10 mg plus metformin,
66.3% for saxagliptin 10 mg, and 68.4% for metformin.
There were just five cases of confirmed hypoglycemia, three of
which occurred in patients taking saxagliptin 10 mg plus metformin,
while the remaining two occurred in patients taking metformin.
Pfützner concluded that “the combination of saxagliptin and
metformin may be an attractive first-line treatment option for Type
2 diabetes.”
